Patient History
70 year old man admitted with heart failure

Findings
Extensive enhancement near transmural with subendocardial involvement in the anterior wall, apex, and septum in the Left anterior descending ( LAD) coronary artery distribution. There is also a low-signal intensity non enhancing structure attached to the LV apex consistent with a thrombus. The left ventricular ejection fraction is severely reduced. The apical segments are dyskinetic.

Impressions
Ischemic cardiomyopathy. Non viable LAD infarction with apical thrombus formation.
