Patient with suspected apical pathology referred for Cardiac MRI

Findings
There is thickening of the LV apex, diffuse subendocardial enhancement in the apical segments extending into the myocardium and a non enhancing low signal intensity structure representing a thrombus attached to the LV apex.

Impressions
Eosinophilic endomyocarditis

Findings
The muscle of the LV apex is disproportionately thickened compared to the base and apex to base thickness ratio > 1.5. The LV cavity has a spade like shape. Diffuse enhancement is present in the LV apical myocardium

Findings
Hypertrabeculated non compacted myocardium is seen at the LV apex with a ratio of non compacted to compacted myocardium greater than 2.3.

Findings
There is enhancement in the LAD distribution involving the apical anterior wall, apical cap and apical inferior wall and affecting the subendocardium. A low signal intensity non enhancing structure representing a thrombus is attached to the LV apex
