Case: Epidural Hematoma on CT

Well, in this patient with trauma, we're

gonna start with the scans of the bone

windows, and this is the brain scan.

With the bone windows, we see that the

patient's had a previous zygomatic arch

fracture, as well as nasal bone fractures.

We also see a relatively subtle area where there is a

fracture of the squamous portion of the temporal bone.

This is on the

relatively thick section images.

We can verify that by looking at the thin

section images and identifying this area where

there is a fracture seen on the left side.

So, in the situation where we see

a squamous temporal bone fracture,

we must be concerned about the possibility

of an epidural hematoma secondary to

ripping of the middle meningeal artery.

And, in point of fact, on this axial

brain image, we see a collection.

And if I window this a little bit narrower...

Okay, so now what we see is this collection,

which is centered at the fracture site.

This is going down a little bit lower here.

Centered at the fracture site, it is lenticular.

It's convex towards the brain rather than

being crescentic, and this is what one

would expect of an epidural hematoma.

I'm just gonna use the magnifying capability here

to emphasize that this shape is more in keeping with an

epidural hematoma. So, the fact that it stops at the

coronal suture, the fact that it is convex outward, lentiform

in appearance and shape, and is associated with a

fracture of the squamous portion of the left temporal bone,

suggests that this patient has an epidural hematoma.
