Patient History
35 year old male with complaint of pain of left ring finger after rock climbing
Findings
LIGAMENTS: Discontinuous ulnar aspect of the left 4th finger A2 pulley.
OSSEOUS: No fractures, dislocations or osteoedema.
TENDONS: Mild edema interposed between the flexor digitorum profundus and 4th digit proximal phalanx at the level of the A2 pulley.
GENERAL: Diffuse soft tissue swelling mainly involving the palmar aspect of the 4th finger. Even though the patient has moved, flexion images demonstrate palmar bowstringing of the flexor mechanism as a sign of A2 pulley failure.
Impressions
1.Injury of the A2 pulley mechanism of the left 4th finger with disruption of the ulnar collateral ligament and edema interposed between the flexor digitorum profundus and proximal phalanx.
2.Diffuse soft tissue swelling throughout the 4th finger, mainly involving its palmar aspect.
