Wk 4, Case 3 - Review

Patient History: 76-yo female with constant sharp pain in the right hip, groin, and leg for 3 months.
Findings
SKELETAL: No infiltrative or destructive bone lesion. No acute macrofracture.

PELVIS: No free fluid in the pelvis. No hernia.

JOINTS: Pressure versus marginal erosion of the right femoral neck and femoral head. Mild spurring of the left acetabular rim. Symphysis pubis arthrosis without osteitis. SI joint arthrosis

ACETABULUM/LABRUM: Diminutive degenerated and autodigested right acetabular labrum. Right acetabular spurring.

MUSCLES/TENDONS/LIGAMENTS: Symmetric musculature. Right iliopsoas bursitis. Intact iliopsoas tendon. Enthesopathy at the hamstring origins without tear. Mild right gluteus medius and minimus tendinosis and peritendinitis without macrotear.

OTHER/SOFT TISSUE: No adenopathy. Degenerative joint and disc disease in the included lumbar spine.

Conclusion
1. Severe complex capsulosynovitis compatible with synovial chondromatosis. Resulting pressure versus marginal erosions of the femoral neck and head. Right acetabular labral tearing and autodigestion. No AVN, stress fracture, or macrofracture.
2. Complex right iliopsoas peritendinitis, also with synovial chondromatosis.
