Coronal Anatomy: Extrinsic Ligaments Part 11

HIDE
PrevNext

0:01

Wrist anatomy, extrinsics, dorsal. We've got a

0:06

horizontal V shape for the key dorsal ligaments known

0:11

as the dorsal intercarpal ligament right here, and

0:15

the dorsal radiotriquetral ligament right here.

0:18

Of the two, this one is probably a

0:20

little more important, the distal one.

0:23

I point out this configuration of a sideways V because

0:26

on the volar aspect of the wrist, and this is how you

0:28

tell where you are very quickly, the ligaments orient

0:32

themselves in an inverted V, from distal to proximal.

0:36

So when you see a sideways V,

0:37

you're on the back of the wrist.

0:39

When you see a V pointed from front to back or

0:43

from distal to proximal, kind of like an arrow,

0:46

you know you're on the volar aspect of the wrist.

0:50

Now the dorsal intercarpal ligament goes

0:52

from the dorsal tubercle of the triquetrum

0:55

here to the dorsal groove, remember,

0:58

on the back of the wrist of the scaphoid right here.

1:03

This ligament also has attachments to the lunatotriquetral

1:06

ligament and to the scapholunate ligament.

1:11

It also attaches to the lunate itself directly,

1:15

and about 50% of the time, it may also have

1:17

some attachments to the adjacent multangular bones,

1:22

although that's of lesser importance and significance.

1:25

The attachments to the SL and LT ligaments is important

1:29

because patients who have scapholunate dissociation

1:33

frequently have rupture of one or both

1:36

of these ligaments, especially this one,

1:38

the dorsal intercarpal ligament.

1:42

Let's have a look at one or

1:43

both of these ligaments on MRI.
