39-yar-old man with pain and lump in the index finger. History of pinching injury one year ago.

Long- and short-axis fat- and water-weighted images were performed.

CASE FINDINGS
A 1cm x 0.9cm x 0.8cm loculated lesion with T1 and T2 hyperintense content and a tail-like communication with a thin linear structure at the radial aspect of the middle phalanx of the index finger most likely representing a digital artery pseudoaneurysm. Otherwise the visualized osseous structures tendons and ligaments of the PIP and DIP joints of the index finger are normal.
CASE CONCLUSION
A 1cm x 0.9cm x 0.8cm loculated lesion at the radial aspect of the middle phalanx of the index finger most likely representing a digital artery pseudoaneurysm. Duplex ultrasound study is recommended to confirm flow within the lesion but epidermoid schwannoma and unlikely myxoma or adventitial cysts are considerations to be subsequently excluded with contrast MRI.
