Hip – Capsular rupture

CASE HISTORY

42-year-old female with history of hip surgery complaint of left hip pain with weightbearing for the last several months. History of arthroscopy and suspected labral repair.

TECHNICAL FACTORS

Multiplanar imaging was performed prior to and following intraarticular (or intravenous) injection of gadolinium solution. 0.1 cc of Optimark injected.

CASE FINDINGS

• Internal fixators are noted in the iliac bone. Associated magnetic susceptibility artifact partially obscures the imaging field-of-view and visualization of the joint space. 
• Interval repair of the superolateral labrum with evidence of bioresorbable screw tracts. Shallow acetabulum along with chronic tear of the hypertrophic superolateral labrum are again noted. Partial resection of the anterosuperior femoral head and a 1cm in diameter iliofemoral ligament defect at the superolateral aspect along with a 1.5cm x 1cm capsular fluid collection beneath the gluteus minimus are seen. Interval development of moderate effusion. No free fragment identified within joint capsule. Ligamentum teres appears intact. Transverse ligament is unremarkable. No displaced retear of labrum. Moderate chondral thinning at the weightbearing surfaces along with interval development of full thickness 1cm in diameter chondral erosion at the superolateral aspect are noted. 
• Tendons about the left hip are unremarkable.

CASE CONCLUSION

• Interval repair of the acetabular labrum partial resection of the anterior-lateral aspect of the femoral head and a 1cm in diameter iliofemoral ligament defect at the superolateral aspect along with a 1.5cm x 1cm capsular fluid collection beneath the gluteus minimus. Moderate capsulosynovitis. 
• No new displaced labrum tear identified. 
• Moderate chondral thinning at the weightbearing surfaces along with interval development of full thickness 1cm in diameter chondral erosion at the superolateral aspect.
