CASE HISTORY
Medial ankle pain. Patient working out and jumping. Medial pain and some swelling.

Long- and short-axis fat- and water-weighted images were performed.

CASE FINDINGS
- Microtrabecular injury of the cuboid is noted and may correlate with the patient’s clinical syndrome.
- The peroneus longus and brevis are intact.
- The medial tendon group is intact. The Achilles is intact. Small ankle effusion. Intact major ligaments.
- As a source of medial ankle pain careful inspection of the deltoid is undertaken which remains intact. Laciniate ligament is unremarkable.
- A small but definite osteochondral erosion with chondral irregularity of the superomedial talus is noted.
CASE CONCLUSION
Microtrabecular injury of the lateral cuboid but correlating with the patient’s clinical syndrome is an osteochondral erosion of the superomedial talus associated with prominent tumefactive synovial thickening and in view of the absence of pigment PVNS would appear a less likely consideration.
