Associated Features on MRI

So I want to talk about some associated

features that we can see on MRI, and these

include things like nipple retraction, nipple

inversion, skin retraction, and skin thickening.

We may see skin invasion, where the breast

tumor directly invades the skin, or there's

inflammatory breast cancer where there's

actually metastatic disease within the skin.

We can see axillary adenopathy, pectoralis

muscle invasion, chest wall invasion,

and associated architectural distortion.

I wanted to show you a few of these features

and then we'll go over a case that has a

lot of these features associated with it.

So this is a patient

who has inflammatory breast cancer and

her breast skin is diffusely thick, and

her nipple is actually retracted here.

So the nipple, the shape of the

front of the breast is a little bit

deformed and her nipple is pulled in.

This is a patient who has a breast cancer

that's involving her pectoralis directly, almost

extending to the back of the pectoralis muscle.

So pectoralis involvement.

And here's a patient who

has a large breast cancer.

It actually extends from the skin to the

pectoralis muscle, but it doesn't stop there.

It goes to the chest wall.

So that yellow arrow is

indicating chest wall enhancement.

You can see that that's pretty diffuse.

So sometimes we'll have a request to look at

breast MRI when we have kind of a larger mass

or a fixed mass, it's already been biopsied,

we know it's a breast cancer, but we're trying

to figure out what structures are involved

and how can we help the surgery team and

treatment team better plan for treatment.
