Paraspinal Extramedullary Hematopoiesis

When we consider the diagnosis of

extramedullary hematopoiesis,

we have to recognize that one of its manifestations

is paraspinal masses in the posterior

mediastinum. This was a chest CT scan that really

a neuroradiologist shouldn't be looking at.

However, when we evaluated the patient for

potential spinal pathology,

we started note that there were nodules that were

present in a paraspinal location bilaterally at multiple

levels that were not extending into the neural foramen.

So not a CNS process,

but as you can see, a multilevel,

multilobulated process that is predominantly in a

paraspinal location. On the coronal

reconstructed images,

you can get a better sense of the multiple nodular

paraspinal processes that do not extend into the neural foramina.

And this was an example of a patient who had

extramedullary hematopoiesis as part of a manifestation

of a myelodysplastic syndrome.
