This patient's a 32-year-old who's postpartum.
0:04
She'd had retained products of conception
0:06
related to this pregnancy twice and then
0:08
represents with recurrent bleeding again.
0:11
So here's our sagittal imaging of the
0:12
uterus, and you can see lower down,
0:15
we have echogenic thin endometrial stripe.
0:19
We have a measurement there around
0:20
eight millimeters or so, but as we keep
0:23
scrolling, it gets thicker and thicker.
0:24
And again, here's part of the stripe here is
0:26
it pushed aside by this structure back here.
0:29
And again, an anechoic tubular
0:31
and somewhat rounded structure
0:33
as well in this particular area.
0:36
So she'd already gone to the operating
0:37
room twice for retained products of
0:39
conception, and then we found this
0:42
structure here with her recurrent bleeding.
0:44
We did do color Doppler flow on this,
0:46
and you can see this completely fills in
0:51
right here.
0:52
So we were concerned here that this was actually
0:54
a true AVM in her particular case and we weren't
0:58
quite sure all of this material right here in
1:00
the endometrium. Is this going to be recurrent
1:03
retained products of conception for the third
1:05
time, or is this just going to be hematoma?
1:07
And given the presence of this, which
1:09
wasn't there on her prior ultrasound
1:11
when she was diagnosed with the retained
1:12
products of conception, our gynecologist
1:15
colleagues didn't want to take her back to
1:16
the operating room a third time with this.
1:19
So they pushed her towards CVIR intervention,
1:22
where we're going to embolize this.
1:23
But for embolization for IR, you cannot
1:27
have retained products of conception,
1:28
because if you do and you embolize
1:29
that, the patient can become septic.
1:32
It's also important to note here.
1:33
Here is our low resistance waveform when
1:35
we put our spectral Doppler on here.
1:37
In her case, it wasn't particularly high.
1:40
If it's greater than 80 centimeters
1:42
per second, that is more suggestive that
1:44
this is not going to go away on its own.
1:47
And this one wasn't that high,
1:48
but we got it up to around 50 or so.
1:50
But again, she was bleeding and she
1:51
had contraindications for surgery.
1:53
So again, jumping back, we need to make
1:55
sure before they go to embolization
1:57
that we're absolutely sure this is
1:58
not retained products of conception.
2:00
And you can see on here, there really
2:01
isn't much vascularity here, so we're
2:03
pretty sure this was all just blood clot.
2:05
But if we could be as sure as
2:06
possible, that's what we wanted to be
2:07
before sending her to embolization.
2:09
So what we did is we gave
2:10
a little bit of contrast.
2:12
So this is your grayscale image over here.
2:14
It's going to be a low B mode.
2:15
So the images are not as pretty, and this
2:17
is going to be your contrast imaging.
2:19
So anything that's bright is enhancing, except
2:21
for your hard acoustic interfaces here, which
2:23
are going to be intrinsically bright here.
2:25
So we'll inject the contrast.
2:27
You can see again, she's breathing a little bit.
2:28
We're starting to sweep a little bit,
2:33
and you can see the contrast bubbles
2:34
start to come in and they fill that cystic
2:37
area very, very quickly, very quickly.
2:40
So we thought this was most likely
2:41
going to be an AVM. And as we keep going,
2:43
we keep sweeping through.
2:46
The uterus itself enhances as expected,
2:48
and then we start to get to the endometrial stripe
2:50
here and you can see we're in that area of
2:52
heterogeneity, right there, the stripe there.
2:54
This is some of the
2:55
heterogeneous portions of it.
2:57
And it's not enhancing.
2:58
If you have retained products of conception,
3:00
I would expect that to enhance because
3:02
that is a vascularized material,
3:05
whereas a hematoma is avascular and
3:07
you can see here, our AVM is
3:10
poking in a little bit to that
3:11
endometrial canal, but none of the
3:13
material in the endometrium is enhancing.
3:15
We could confidently diagnose that
3:17
this was all just blood product.
3:19
She did not have recurrent retained products of
3:20
conception, but she did have what is
3:22
most likely a vascular malformation.
3:25
So she did end up going to interventional
3:27
radiology and successfully embolized.
3:30
And you can see on some of her post-
3:31
contrast imaging a few weeks later,
3:35
her endometrium was great, nice and thin,
3:37
maybe a few little calcifications,
3:40
but that vascular lesion is now gone.
3:42
Her bleeding had stopped,
3:46
and so we were just going to continue to
3:47
watch her for a little bit and counsel
3:49
against getting pregnant again until we're
3:52
absolutely sure everything had resolved.
3:54
But this was, she was embolized
3:56
with gel foam to preserve fertility.
3:58
It's also probably going to account
3:59
for some of these little echogenic
4:00
foci that we're seeing, not all
4:02
necessarily going to be calcifications,
4:04
but the result of the embolization products.
