<div>HISTORY:&#160;This 66-year-old male is being evaluated for a right distal thigh mass.(QUIZ ANSWER) TEAR IS IN:&#160;Quadriceps tendon.Using the&#160;diagnostic web viewer, we have provided images that assist in telling our clinical story. Areas of significance are indicated below.FINDINGS: Status post total knee replacement. No acute bony macrofracture or contusion in the distal femur or the patella. Chronic moderate-severe tendinopathy of quadriceps tendon with a greater than 7cm in length and maximum 1cm in width full thickness split tear, VLO (vastus lateralis) segment. Moderate sized complex effusion with aggregated areas of hypointensity superolaterally. No acute muscle strain.&#160;CONCLUSION:&#160;1. No mass. A large joint effusion with inflammatory pseudotumor.&#160;2. A full thickness longitudinal split tear of quadriceps tendon, VLO (vastus lateralis) segment.&#160;3. Status post total knee replacement.&#160;4. Moderate sized complex effusion with aggregated areas of hypointensity superolaterally. Differential diagnosis includes metallosis; ALVAL (acute lymphovascular reaction); and reaction to multiple intra-articular bodies/ synovial metaplasia. Favor metallosis.</div>
