Case 3 - 46-year-old female presents with history of fibroids and pelvic mass

<div>HISTORY:&#160;This 46-year-old female presents with history of fibroids and pelvic mass.(QUIZ ANSWER) MOST LIKELY PRIMARY FINDING:&#160;Carcinoma.Using the&#160;diagnostic web viewer, we have provided images that assist in telling our clinical story. Areas of significance are indicated below.FINDINGS: The liver, biliary tree, gallbladder, pancreas, spleen, and adrenals are unremarkable. Few small cysts are present in the kidneys.&#160;Few small para-aortic and paracaval lymph nodes. No evidence of upper abdominal lymphadenopathy. No evidence of bowel obstruction or inflammation.&#160;The bladder is unremarkable. The uterus appears unremarkable, without evidence of mass. The right ovary is somewhat small but otherwise normal. However, the uterus is displaced to the right due to a heterogeneously enhancing 9.7 x 8.2 x 9.5cm left pelvic mass that is contiguous with the left ovarian vein. This is consistent with a left ovarian tumor. Given the size and heterogeneous enhancement, ovarian cystadenocarcinoma is a concern. Exact pathology uncertain.&#160;No large or substantive pelvic lymphadenopathy noted. The sigmoid is redundant.&#160;CONCLUSION:&#160;1. Large left pelvic / ovarian mass with heterogeneous enhancement concerning for carcinoma.&#160;2. Resulting mass effect with displacement of the uterus to the right. No evidence of mass or discrete myoma in the uterus. The right ovary appears unremarkable.</div>
