HISTORY:
This 49-year-old female presents with injury of the left wrist with limited range of motion and swelling. Patient fell while running, and has ulnar-sided pain and failed anti-inflammatory therapy.

Using the diagnostic web viewer, we have provided images that assist in telling our clinical story. Areas of significance are indicated below.
FINDINGS:
An effusion is present in the distal radioulnar joint and in the palmar TFCC recess.
The 3D coronal T2 fast field echo images demonstrate a traumatic perforation best, and has likely led to the patient's effusion.
The scapholunate and lunatotriquetral ligaments are both intact.
The extensor carpi ulnaris is intact demonstrating normal position in the radioulnar groove.
CONCLUSION:
1. Perforation of the central third of the TFC likely traumatic as a tear with surrounding synovial inflammation and effusion in distributions as described but without rupture of the lunatotriquetral ligament or peripheral TFCC attachments.
2. ECU intact.
