16-year-old girl for 6 month follow up of probably benign mass in right breast

0:00

And our next patient is a 16-year-old girl

0:04

who is being seen for six-month follow-up of

0:06

a probably benign mass in the right breast.

0:10

So she had come in with a palpable area

0:12

of concern that was evaluated and either

0:15

declined biopsy or wanted to wait.

0:18

And we agreed to do a six-month follow-up.

0:21

So that's what she's here for now.

0:24

So looking back at the previous

0:27

exam, we see this mass.

0:30

It's labeled at right breast, 1130,

0:33

8 centimeters from the nipple, palpable.

0:35

And we can see this oval hypoechoic mass.

0:39

And it had internal blood flow and measured

0:48

2.4 centimeters in greatest dimension

0:53

by 1.0 by 2.0 centimeters.

0:55

So it's about a two-centimeter mass.

0:59

So she returns at six months for

1:02

follow-up, and this is what we have.

1:05

Now our mass is over four centimeters.

1:08

So this is in, you know,

1:10

a relatively short period of time.

1:11

She was supposed to come back at six months.

1:13

She came back at more like

1:15

10 months, but now the mass is 4.

1:18

3 centimeters by 1.6 by 2.8 centimeters.

1:24

So this is quite a large increase over time.

1:28

And at this point, we're now

1:30

going to recommend biopsy.

1:32

And because it's sort of,

1:33

um, rapidly enlarging, even if we get a

1:36

benign result, we're probably going to suggest

1:38

surgical consultation for possible excision.

1:42

So um, this was recommended for biopsy.

1:44

So it was a BI-RADS 4 and biopsy was performed

1:48

and the result was a cellular fibroadenoma

1:53

and surgical excision was subsequently

1:56

performed, um, with the same diagnosis.

1:58

So no upgrade.

2:00

It was a cellular fibroadenoma,

2:02

but quite rapidly growing.
