28-year-old woman, 1 year postpartum, with palpable lumps right breast

Our next patient is a 28-year-old woman.

0:03

She's one year postpartum, um, still breastfeeding

0:08

with palpable lumps in the right breast.

0:12

So the patient indicated the palpable areas

0:14

of concern, and one of them was in the

0:17

right breast at the six o'clock position.

0:19

When we did an ultrasound in

0:21

that area, we found this mass.

0:23

So, relatively hypoechoic mass with

0:26

irregular margins and some internal echogenic

0:30

foci, which are probably calcifications.

0:33

You can see them here in the other plane.

0:35

So, this is antiradial and then

0:37

radial, fairly large mass, 3.4 centimeters.

0:41

16 00:00:43,420 --> 00:00:46,510 And then the second palpable area was in the

0:46

right breast at the nine o'clock position.

0:48

We could see this oval mass with a fluid

0:52

debris level and the debris that's actually

0:56

floating in a non-dependent portion of

0:58

this lesion is the more echogenic material.

1:02

So this is actually a classic appearance

1:05

of a galactocele with a fat fluid level

1:09

and the fat is floating in a non-dependent

1:11

position because it's lighter than the fluid.

1:13

So this was a galactocele.

1:17

And we also looked at the right axilla,

1:21

thought her lymph nodes looked okay.

1:23

So this, this mass in the right breast at

1:25

the six o'clock position was suspicious

1:28

and was recommended for biopsy.

1:30

And that was found to be a poorly

1:32

differentiated invasive ductal carcinoma.

1:35

And the other mass was a galactocele.
