Inflammatory Breast Cancer Overview

HIDE

0:00

So our next topic is inflammatory breast cancer

0:04

which can sometimes be difficult to

0:06

distinguish from, um, mastitis or some

0:10

of these more chronic forms of mastitis.

0:12

So in inflammatory breast cancer,

0:14

there are rapid changes in the skin overlying

0:16

the affected breast with erythema, edema,

0:19

or "peau d'orange," orange peel kind of skin.

0:24

And that develops usually over

0:26

the course of about 3 to 6 months.

0:28

The skin changes usually affect

0:30

at least a third of the breast.

0:32

There should be pathologic evidence of

0:34

invasive breast cancer in order to prevent it,

0:37

called inflammatory breast cancer.

0:39

And this represents 2% to 4%

0:41

of all breast cancers.

0:43

And the histopathology shows scattered

0:45

tumor emboli and dermal lymphatic invasion.

0:48

There's a higher rate of HER2

0:51

and triple-negative subtypes.

0:53

Those skin changes can be mistaken for

0:55

mastitis, so this can be a delayed diagnosis.

0:59

And diagnostic clues include a little

1:02

bit older age, age over 50,

1:05

diffuse erythema, and no systemic symptoms.

1:08

So these patients often have all these

1:12

changes, but it's relatively painless

1:15

without the symptoms of fever and chills.
