Emergency Imaging: Chest

Exam: XR Chest 1 View Portable
CLINICAL INDICATION: Chest pain 
COMPARISON: Most recent prior.
FINDINGS/IMPRESSION:  
Cardiac and mediastinal contours are normal. RUL consolidation. No edema,
pleural effusion or pneumothorax. No destructive bone lesions.


Exam: CT Chest w/ Contrast
CLINICAL INDICATION: Empyema
TECHNIQUE: Written informed consent was obtained. Non-gated spiral axial images
of the chest were obtained with nonionic intravenous contrast. There were no
immediate complications reported.
 FINDINGS:
MEDIASTINUM/HEART/VESSELS: 
There are prominent mediastinal lymph nodes which likely are reactive and have
increased in size. The thoracic aorta is normal in size. There is no evidence of
dissection. The heart size is normal. The pericardium is normal. 
AIRWAY/LUNGS/PLEURA:
There is worsening right upper lobe consolidation. Additionally, there are tree
in bud opacities in the right middle and lower lobes consistent with
endobronchial spread of infection. There are a few small nodules on the left
which also appear to be new and likely are infectious. There is a small right
pleural effusion. There is no pleural enhancement to suggest empyema.
VISIBLE ABDOMEN:
There were cysts in the left kidney. Images of the upper abdomen are otherwise
unremarkable.
SOFT TISSUES/BONES:
There are no aggressive osseous lesions.
IMPRESSION:
1. Worsening bronchopneumonia. No evidence of empyema.
