<div>HISTORY:&#160;This 62-year-old male presents with paralysis of the left index finger and thumb, and deep numbness since six months prior. Patient also complains of pain in the chest and left arm.(QUIZ ANSWER) PRIMARY DIAGNOSIS IN THIS CASE:&#160;Kiloh-Nevin syndrome.Using the&#160;diagnostic web viewer, we have provided images that assist in telling our clinical story. Areas of significance are indicated below.FINDINGS: No recent macrofracture or bony contusion. Visualized aspects of the elbow joint spaces are intact. Mild capsular fluid is noted. Undersurface microtear of the common extensor tendon is noted. Mild tendinopathy of the common flexor tendon.&#160;No soft tissue mass identified.&#160;Generalized myoedema of the flexor pollicis longus, flexor digitorum profundus, pronator quadratus without fatty infiltration or volumetric atrophy suggestive of anterior interosseous nerve syndrome. However, myoedema of flexor carpi ulnaris and flexor digitorum superficialis are also noted. Therefore, brachial plexopathy is also in the differential considerations.&#160;Mild myoedema of the median nerve in the carpal tunnel is noted.&#160;No retracted tendon tear or evidence of compartment syndrome.&#160;Moderate degenerative arthritic changes of the ulnocarpal joint along with synovitis of the TFCC.&#160;CONCLUSION:&#160;1. Generalized myoedema of the flexor pollicis longus, flexor digitorum profundus, pronator quadratus without fatty infiltration or volumetric atrophy suggestive of anterior interosseous nerve syndrome - Kiloh-Nevin syndrome. However, myoedema of flexor carpi ulnaris and flexor digitorum superficialis are also noted. Therefore, brachial plexopathy is also in the differential considerations.&#160;2.
