Case 6 - 49-year-old male presents with right hip and groin pain

<div>HISTORY:&#160;This 49-year-old male presents with right hip and groin pain for several months. WHAT IS NOT A FINDING:&#160;Abnormal sacroiliac articulations.Using the&#160;diagnostic web viewer, we have provided images that assist in telling our clinical story. Areas of significance are indicated below.FINDINGS: Osseous structures appear grossly intact and exhibit normal overall marrow signal. Sacroiliac articulations and pubic symphysis are normal in appearance. The musculature is symmetric and normal in appearance.&#160;Intermediate grade degenerative chondromalacia is identified involving the right hip articulation, accompanied by 7mm subchondral arthropathic cyst involving the acetabulum. Far anterolateral acetabular subchondral cystic change is also demonstrated, accompanied by nondisplaced far anterior to posterosuperior labral tear. Far posterior acetabular and ischial osseous stress response is noted. Focal subchondral arthropathic cyst is also identified involving the superolateral aspect of the left acetabulum. Focal cystic change is identified in the distribution of the anterolateral aspect of the right femoral head-neck junction, indicative of cam-type femoroacetabular impingement.&#160;Discogenic spondylosis is identified at L4-L5 with associated right eccentric sterile endplate fibrovascular inflammatory response at L4.&#160;CONCLUSION:&#160;1. Intermediate grade right hip degenerative chondromalacia with acetabular subchondral arthropathic cyst and far anterior to posterosuperior nondisplaced labral tear.&#160;2. Focal cystic change involving the far anterolateral aspect of the right femoral head-neck junction, indicative of cam-type femoroacetabular impingement.&#160;3. Far posterior acetabular and ischial osseous stress response.&#160;4. L4-L5 discogenic spondylosis accompanied by right eccentric L4 sterile endplate fibrovascular inflammatory response.</div>
