<div>HISTORY:&#160;This 64-year-old male presents with palmar pain in left hand primarily around the thumb. Limited range of motion. No history of prior surgery.JOINT SPACE NARROWING AT THE BASE OF THE METACARPAL OF:Thumb.Using the&#160;diagnostic web viewer, we have provided images that assist in telling our clinical story. Areas of significance are indicated below.FINDINGS: Joint space narrowing at the base of the metacarpal of the thumb as it articulates with the greater multangular with chondromalacia on both sides of the joint, in addition to osteophytes and surrounding soft tissue swelling. 3mm penetrating erosion at the base of the metacarpal with smaller 2mm similar such focus within the greater multangular.&#160;Radiocarpal joint space narrowing with 4mm subchondral cyst of the radius. Intrinsic signal intensity of the osseous structures and joint spaces elsewhere well maintained.Triangular fibrocartilage intact. Mainly the attachments onto the radius, lunate, and triquetrum are unremarkable.Scapholunate and lunatotriquetral intervals as well as the ligaments both dorsally and volarly are intact.Tear of the oblique, beak ligament.&#160;Extensor and flexor tendons unremarkable throughout.&#160;CONCLUSION:&#160;Arthropathic changes at the carpometacarpal joint space of the thumb with surrounding soft tissue swelling. Mild arthropathic changes at the radiocarpal joint space as well. Tear of the oblique, beak ligament. Adductor pollicis space swelling could contribute to adductor entrapment.</div>
