Case 10 - 18-year-old male presents with sharp pain in right elbow

<div>HISTORY:&#160;This 18-year-old male presents with constant sharp pain in right elbow since fall with all of body weight onto elbow.(QUIZ ANSWER) PRIMARY FINDING:&#160;"Sloppy hinge" syndrome.Using the&#160;diagnostic web viewer, we have provided images that assist in telling our clinical story. Areas of significance are indicated below.FINDINGS: Sloppy hinge syndrome with moderate to advanced osteoarthritis. Hyperextended elbow. Irregular defects of lateral and posterior capitellum with moderate-severe osseous edema. Evidence of common extensor tendon repair, chronically thickened. No recurrent macrotear. Subtle depression at articular surface of anterior radial head. Severe chondral thinning and mild osseous edema of peripheral aspect of radial head. Mild lateral subluxation of the radial head.&#160;Low-grade osseous edema at the top of the olecranon. Foci of osseous erosion at base of the olecranon. Articular internal olecranon spur. The ulnar collateral ligament and common flexor tendon intact.Biceps and triceps tendons intact.&#160;No acute muscle strain. The ulnar nerve is normal in caliber.&#160;IMPRESSION: Right Elbow: Sloppy hinge syndrome with moderate to advanced osteoarthritis.Deformed capitellum with moderate-severe osseous edema. Mild depression at radial head articular surface and severe chondral thinning. The findings probably from a combination of old and new trauma of valgus injury with subtly impacted fracture of the radial head and capitellum, superimposed with post-surgical change.&#160;&#160;Status post common extensor tendon repair, no recurrent macrotear.&#160;Low-grade contusion of the olecranon.
