Case 21 - 44-year-old male presents with right elbow pain

<div>HISTORY:&#160;This 44-year-old male presents with right elbow pain.(QUIZ ANSWER) DOMINANT FINDING IN THIS CASE:&#160;Edematous and swollen ulnar nerve.Using the&#160;diagnostic web viewer, we have provided images that assist in telling our clinical story. Areas of significance are indicated below.FINDINGS:&#160;Ligaments: The anterior and posterior band of the ulnar collateral ligament is normal. The lateral ulnar collateral ligament is also intact.Tendons: The brachialis, biceps and triceps tendon insert normally. Moderate tendinopathy of the common extensor tendon is present. No disruption is identified. Moderate soft tissue swelling is identified along the medial aspect of the elbow and this includes minimal tendinopathy of the common flexor tendon. No disruption is identified.&#160;Bone Marrow: No AVN or fractures are identified.&#160;Soft Tissues: No joint effusion is identified. Soft tissue swelling is identified along the medial aspect of the elbow where an ulnar transposition has been performed. The course of the ulnar nerve is identified. The ulnar nerve has a fasciculated and swollen appearance. This swollen appearance begins proximal to the elbow joint and continues&#160;until it reaches its normal anatomic location in the forearm musculature distal to the elbow. Soft tissue swelling is identified around the ulnar nerve including much of the musculature in the flexor group. However, no space-occupying mass is identified. No abscess is identified. No discontinuity of the ulnar nerve is identified. A fiducial marker has been placed along the posterior aspect of the elbow joint distal to the anconeus muscle. No mass or abnormality is noted in this location.&#160;CONCLUSION:&#160;1. In the area of interest, in the posterior aspect of the elbow&#160;no abnormalities are noted.&#160;2. The patient is status post ulnar nerve transposition.&#160;3. The dominant finding is an edematous and swollen ulnar nerve&#160;4. Diffuse soft tissue swelling as edema is identified in the flexor muscle mass, consistent with prior surgery. No space-occupying mass such as a hematoma or abscess is noted along the course of the ulnar nerve and the ulnar nerve, although swollen, is intact.&#160;5. Moderate-to-severe common extensor tendinopathy with mild common flexor tendinopathy.</div>
